Without any other choice possible but to be spanked each three times you don't comply, the player is led too wear full female clothing (even in gym). However, complying has the exact same result: you end up caged and getting dominated by a woman (the storyteller, probably). At this point, is it still a game? As you only need to follow the story passively and answer what you've been told, it looks more of a series of tasks. And you're not supposed to try anything but that. And not supposed to have fun either. Just shut up and learn to be a sissy. Let me think: this game is called Schizophrenia. You where supposed to have a schizophrenia, not knowing if you feel like a boy or a girl, right? And you are led to be a sissy, forever in between both, with a submissive attitude which was not deemed. This clinic is a scam, this doctor is a scam, this game is a scam, this dev is a scam. Check if you're not a scam yourself after playing.

You asked for feedback? Here's mine. You try lead the player to your own aim with no tact. For example, you use a lie detector to help you obtaining the answers you need to pursue your own, exact version story. How it works is clear: the player cannot continue the story until he finally gives ALL the answers you wish. Such a psychology level is close to zero. You only act forceful and give no envy to follow you. You only mean to obtain a submissive, ready to be dominated sissy. Any resistance is rejected until the player fully agrees or quits. In fact, you don't accept the complex psychology of a man when he feels more or less feminine. And you probably don't care. IMO, you don't need publishing a game to fulfil your pleasure. Just write your story the way you want it, and as you don't give the players any sort of decision you basically don't need them.
